The PREVENT Study (PREVENT) respects your privacy. Our policy is that no personal information will be collected about you unless you choose to provide it. Our web server does not record the domain name or Internet Protocol (IP) address of each visitor to our site.

Web Form Policy

The Contact Us page of our website allows you to fill out and submit inquiries through a web form. When you use our web form, we store a record of your entry to address your inquiry.

If you choose to give us information about yourself through an email message or web form, we keep the information only as long as needed to respond to your question or to fulfill the stated purpose of the communication.
